In addition to the forms, include a separate page on which you state the following:

･ Your First and Last Name

･ Your Driver's License Number

･ The state in which your driver's license was issued

･ Your date of Birth

･ Your Citizenship (if not the United States, include your passport number).

When you have completed your Landing Permit application and your Hold Harmless forms, and signed them in blue ink, send them along with the personal information listed above to:

If you have a (color) scanner available you may optionally print the forms, sign them (in blue ink), scan them and attach to an email to [email protected].

All forms and personal information must be received by Dyess no later than April 2, 2015

Here's an email I sent to my insurance agent, as an example:

"Hi Bill,

I'll be racing at Dyess Air Force Base on 2 May this year, and they need to have a completed DD Form 2400 from you by 2 April.

I've attached a blank form in case you don't have a DD2400 handy.

Regardless what instructions and address is printed on the form, please send your original - signed in blue ink - directly to Dyess Air Force Base:
